SHMGHealth is the official patient portal for Santa Clara Valley Medical Center’s entire network. It’s not some third-party app slapped on top (it’s) built into the county’s system.
I use it. My neighbor uses it. So do the folks at the behavioral health clinic down on Story Road.
It’s different from MyChart or other generic portals because it connects directly to county-run services. Including safety-net clinics and mental health programs. That matters.
A lot.
Secure messaging with your provider is one of the four core things it does. You can also view lab and imaging results, schedule appointments, and check your meds list for accuracy.
It is not telehealth. You still need to show up for video visits (or in person). It won’t bill your insurance.

Go to shmghealth.org. Not .com. Not .net. .org.
Type it yourself (don’t) click a search result. (Google ads lie. I’ve seen it.)
Click “Sign In.” Then click “First Time User.” Don’t skip that. It’s not optional.
You’ll verify identity with ID + DOB + last 4 of your SSN. Or use alternate verification if you’re uncomfortable with that. I prefer the alternate method (less) exposed data, same access.
Password rules? Eight characters. One uppercase.
One number. No “password123.” Seriously. I’ve reset that one myself.
Two-factor: SMS works. But an authenticator app is safer. SMS can be hijacked.
Your phone isn’t always yours (ask) anyone who’s lost one.
The dashboard has five main tabs: My Appointments, Test Results, Messages, Medications, Documents. That’s it. No mystery menus.

I log in to my portal at least twice a week. Not to check lab results (though I do that too). To do things.
The “Message Your Doctor” button? Don’t type “I feel weird.” Tell them when the headache started. List every pill you took yesterday.
Including that ibuprofen at 3 p.m. And never use it for chest pain or sudden weakness. That’s a call to 911.

Go to Health Information, then click “Export All Records.” You’ll get one PDF with visit summaries, immunizations, and meds. Save it somewhere you’ll actually find it later (not Downloads).
Test results show as “pending” until the clinic reviews them. “Released” means your doctor signed off. If it’s been more than 7 days past your blood draw and it’s still pending? Call.
Don’t wait.
Update your phone number right there in the portal. Add proxy access for your kid or aging parent. Spot a typo in your allergy list?
Fix it yourself. No faxing required.
Bookmark the portal homepage. Turn on browser autofill for login fields. Small thing.
Saves 20 seconds every time. That adds up.
